We have revised our semen collection and storage contract to more clearly state the rights and responsibilities of both you and C.L.O.N.E. West. A copy of the new contract, entitled C.L.O.N.E. West Semen Freezing, Storage and Shipping Agreement (the “New Contract”). 2/1/06 The New Contract is effective immediately and supersedes all previous contracts between you and us. The new contract is in effect even if you have not returned your signed contract. PLEASE NOTE THAT YOUR SEMEN IS NOT COVERED BY ANY INSURANCE AND
WE
ARE NOT RESPONSIBLE FOR ANY LOSS OF SEMEN IN OUR CUSTODY except as stated in
the new contract. With the success of our Frozen Semen Bank, our prior insurance was no longer sufficient to cover the semen stored with us. But, we are pleased to announce that we are now able to introduce you to an insurance carrier that can insure losses related to your frozen semen. This insurance carrier allows you to insure semen for the value you determine. This insurance carrier can also provide insurance coverage for semen during shipment. The private insurance covers fire, vandalism, tank failure and theft while in the custody of Vet. The private insurance does not cover shipping. Separate shipping insurance is available. Earthquake and natural disaster insurance are not available. If you choose not to buy the optional insurance, the coverage provided will allow for recollection of same stud , or if not available, the stud of your choosing, for like number of straws, only in the event that damage or loss results from gross negligence or willful intent of Vet as stated in the contact. Please carefully read the Frozen Semen and Storage Contract. In order for us to store semen for you, we require that you complete, initial and sign it in the places indicated. Semen stored at CLONE West cannot be used or transferred without a signed copy of the new contract on file. A separate New Contract must be completed and signed for each semen donor and, for co-ownerships, a separate New Contract must be signed by each co-owner. An "Additional Information" page can be filled out to specify co-owners. Contracts were mailed to the co-owner whose address was listed when originally collected. If the semen contract is not acceptable to you, a list of frozen semen facilities is available on the AKC web site Please note, if you think that you've decided to move your semen because of the insurance or contract, please carefully screen the semen bank that you choose. We have found that most storage facilities have the same standard coverage as ours. And many do not have third party insurance to refer you to. If
you would like to purchase insurance, please fill out the Insurance Form and
return it with your signed New Contract and payment. Thank
